•Name, address, and contact information for each member of your household to be covered by plan.
•Date of birth and social security numbers for each member of your household to be covered by plan.
•Employer information for each member of your household.
•Income information for each member of your household (for example, wage and tax statements such as pay stubs or W2 forms).
•Information about any other income you receive.
•Names or preferred health care providers and/or medications.
•Connect for Health Colorado and/or PEAK account username and passwords.
