Faith leaders! Please fill out this survey on your experiences with advance care planning conversations.

CCC leaders are invited to participate in a survey to support a Doctorate in Ministry research project.  Dottie Mann is a student from the Iliff School of Theology researching faith leaders’ experiences having conversations about serious chronic illness, medical advance directives and/or end-of-life experiences. 

You are eligible to be in this study because you may identify as a faith leader. A “faith community leader” could be clergy, rabbi, Imam, teacher, volunteer, chaplain, lay leader, choir director, youth leader, spiritual guide, etc. If you decide to participate, please complete the survey at the link below. The first page is the required consent form with all the details. The survey will take about 10 minutes and will be completely confidential. At the end of the survey you will have the option to enter into a drawing for a $50 Amazon gift card.
